With the adoption of the Affordable Care Act in 2010, the idea of quadruple aim emerged. The quadruple aim focuses on improving health outcomes, enhancing the experience of care for individuals, reducing healthcare costs, and ensuring provider satisfaction (Park et al., 2018). There are various measures that need to be taken and various stakeholders that need to be involved in order to attain the quadruple aim. This paper will discuss the components of the quadruple aim in terms of nurse-patient ratio and how the lack of laws in many states regulating staffing can negatively impact all four components of the quadruple aim.

Many nurses have experienced the stress of not being able to do all the required nursing tasks during the twelve-hour shift that they work. Working as a nurse at a hospital with 6 to 1 patient to nurse ratio, I know that I have to be able to plan my day so I can provide my patients with compassionate and the best care I can and, at the same time, to meet my own needs. There are times that most nurses are not able to meet these requirements due to the number of patients that they are assigned to and due to the acuity of individual patients. This in turn results in inadequate patient care and nurse burnout amongst many other issues that will be discussed further.

Although there are many studies demonstrating the negative impact of uncontrolled nurse-patient ratios, there are a few states that have laws and regulations concerning the matter.

According to the American Nurses Association (2019), as of 2019, only 14 states have laws and regulations addressing nurse staffing in hospitals. One study published in 2018, demonstrated a positive correlation between decreased nurse-patient ratio that lead to increased nurse workload and deteriorated quality of delivered care (Qureshi, Purdy, & Neumann, 2018). A recently done study also found that patient safety is being compromised in hemodialysis units with inadequate nurse staffing and a high workload of RNs (Thomas-Hawkins, 2020). Low nurse staffing ratios affect all healthcare disciplines and all the ages of the patient population. Not only adult patients are affected by inadequate nurse-patient ratios, but infants as well. One study found a correlation between higher numbers of the patient to nurse ratio and increased UTI and rotavirus incidents amongst infants (Lee et al., 2020). There are more studies concerning nurse staffing ratios that can be found on the ANAs website under Safe Stuffing Literature Review (ANA, n.d.).

Luckily, I live and work in a state (California) where laws regulating nurse-patient ratios were adopted first. There are specific numbers of patients mandated by law that nurses take care of in various units. There are times when the nurses get sick or have personal matters that make the unit shot of staff. Usually, the nursing office or the unit manager or the supervisor contact people who might be interested in working overtime. When no one is available to work overtime, then the unit is short of staff. This happens on rare occasions but the management and the nursing office exhaust all the options before leaving the unit short-staffed.

Evidence suggests that there are many complications that arise from inadequate nurse staffing in hospitals. Adverse patient outcomes, overwhelmed and burned out nurses, and inadequate quality of patient care to name the few. All these compromises the nurses’ ability to attain the quadruple aim. For these same reasons, it is very important that all states adopt laws regulating nurse staffing ratios.

Workforce retention is a problem in various industries. Retaining experienced nurses is an issue faced by many healthcare entities around the globe. According to Sikka et al. (2015), healthcare systems struggle to meet the triple aim of providing a quality healthcare experience to individuals, improvement in the overall health of populations, and reducing healthcare expenditures. This burden has been balanced on the back of healthcare workers, including nurses, who have been challenged to “do more with less.” Frequently, this has led to burnout and high job turnover rates.

The effect of healthcare workforce dissatisfaction has led to the introduction of a fourth “aim,” focusing on the experiences of those providing care (Sikka et al., 2015). Improving workers’ satisfaction can directly affect each of the other three aims. Healthcare workers feeling dissatisfied has been shown to directly impact patient satisfaction scores as well as measurable outcomes for healthcare systems (Jacobs et al., 2018). Additionally, nurse turnover has a profound effect on a healthcare system’s bottom line. According to Smith (2018), the loss of one trained nurse can cost a healthcare system up to $88,000, in addition to the loss of knowledge and experience. The cost of replacing bedside nurses at one average hospital can run anywhere from $3 million to $6 million annually (NSI, 2020).

While many healthcare systems acknowledge there is a need, few have a definitive plan to address improving staff satisfaction and workforce retention. To survive in the current healthcare environment, leadership in these facilities and systems need to take a more proactive role (Marshall & Broome, 2017). Leaders must acknowledge the interrelated nature of employee satisfaction with patient satisfaction and improved outcomes. Leaders need to promote an atmosphere where employees are respected, appreciated, and are provided the tools needed to learn and grow (Sikka et al., 2015).

I feel that my health care system addresses some areas of workforce retention, but falls short in others. On-boarding and perception are thorough, and there is no lack of opportunity to receive additional training. Additionally, career “ladders” encourage continued growth. There is a robust program for continuing education, including reimbursement for both undergraduate and graduate classes. However, the system continues to experience a high rate of turnover.

One of the concerns that is voiced repeatedly has been staffing. Frequent understaffing, floating, and mandatory on-call shifts place a tremendous amount of stress on nurses. Another issue is the awarding of new hire bonuses without comparable retention bonuses for senior staff. This leaves experienced nurses feeling unappreciated.

Workforce retention in healthcare is a vital part of the provision of quality care. By reducing turnover, healthcare systems that can train and retain skilled staff may reduce costs and improve outcomes and reimbursements. As leaders, we must ensure this issue is addressed in planning for the future of our healthcare systems.
